Patient Contact Update

Verifying patient contact information is such a simple, but such a necessary task. Patients move, they get new phones, and they change jobs. Their information is always changing and they will forget to mention it to you, so it’s up to the front desk to ask the patients.

We recommend making a copy of our form, which you can download below, or making one that better suits your practice. Be sure to put your initials on the form after each patient’s contact info has been updated. This will allow for accountability. Be sure to update this info into your practice management software as the day goes on.

Some offices use digital technology, like iPads to verify information. It doesn’t matter how it is done, as long as it is done!
This information is considered classified, so if you are using paper, be sure to shred it all after you’ve properly updated it in your practice management software.
