  • ACCOUNTS RECEIVABLE (AR)

    Patient accounts/balances.

  • ADA DENTAL CLAIM FORM

    Authorized form for submitting dental insurance claims – be sure to use the most updated/recent version.

  • ADJUSTMENTS

    Changes made to a patient’s account ledger.

  • AGING REPORT

    A report that lists the amounts owed to the practice and is categorized by the number of days late.

  • BACKUP DATA

    An extra copy of data files made at a specific point in time that can be used to restore data to the system in the event data is accidentally lost or destroyed.

  • CHARGES

    The amount(s) a provider bills for services performed.

  • CHART

    A physical or digital place that houses all pertinent patient records.

  • CHART NUMBER

    A unique number that identifies a patient.

  • DENTIST’S STATEMENT OF ACTUAL SERVICES

    A form listing procedures relevant to the specialty of a dental office, used to record the procedures performed.

  • FINANCIAL ARRANGEMENTS

    The way the patient has chosen to pay based on your office financial policies.

  • GUARANTOR

    The person legally responsible for charges.

  • HIPAA

    Federal government act that set forth guidelines for standardizing the electronic date interchange of administrative and financial transactions, exposing fraud and abuse in government programs, and protecting the security and privacy of health information.

  • PATIENT INFORMATION FORM

    Form that includes a patient’s personal, employment, and insurance data needed to complete an insurance claim.

  • PATIENT LEDGER

    A listing of financial activity in a patient’s account.

  • PATIENT PAYMENT

    Payments made to the practice from a patient or a guarantor.

  • PATIENT STATEMENT

    A document that informs the patient of the amount owed to the provider.

  • PAYMENTS

    Monies received from patients and/or insurance carriers.

  • PRACTICE ANALYSIS REPORT

    A report that shows the total revenue for each procedure performed during a specified time period.

  • PROCEDURES

    Work that is done by a provider for a patient.

  • TRANSACTIONS

    Charges, payments, and adjustments posted to patient account ledgers.

  • WALKOUT RECEIPT

    A document listing charges and payment that is given to a patient after an office visit.
